Because it's disappeared. It won't accept an order from customers. my cel: is (415) 734-8795.
I will try to upload another one, but need to know why it disappeared, so I don't have to continually do this.

I see that you had Terms & Conditions widget on the form. This was deleted about 30 minutes ago and should be a manual deletion. Please see the screenshot from revision history of your form.
You may revert this to earlier version that works, from Revision History.
