My embedded form in the website is not working

hello I need to speck with someone about a template I am using asap the app I had at first just came off I redid a new one but now its not fitting the page thank you

When I tried test submitting the form, I could not move forward to the next page of the form even though I have fill-in all the necessary questions on the form.

it was due to other scripts existing in your website that is conflicting with the scripts of the form.

To fix this, please try re-embedding the form in your website using its Iframe embed code.
