    the form was working the day before and the next day the same form is unavailable

    We apologize for the inconvenience. Some of the widgets is causing the error like the one you have on your form "Preview before submit" widget. Our developer has already applied a fix for this. I have tested your form and it no longer redirects to 404 page.
