You are using the source codes and modified some of the elements out of it. You may have accidentally remove some of the important scripts, like the one that validates the required fields. I would advise to re-embed your form again, this time, use our iFrame method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
