Hi,
This is rather odd. I checked to make sure there are no restrictions on your account and there are not. Can you please try clearing your browsers history and cache and logging in again? If that does not work and you are able to try a different browser, please try that also. If you are still unable to create a form, please let us know and we will be happy to assist you further.
