How do I easily add a field to a group catagory?

I what to have various groupings of data like I see on some of the form templates. There isn't one template though that has a grouping exactly as I need it and I can't seem to add a question to a group. For example you might have "mailing address" as one group but in it you would like to add an extra question like appt # and it's not in one of the template forms. When I try to add to that category it just puts it outside the box.

I found an article I think how to add some number but not without writing a lot of code. I don't want to have to do that.

You can use the textbox field if you'd like to ask some other information from your form submitter. Then you can use our Form Collapse to group a few number of fields:
