I believe you're looking for limiting access on your forms by entering their username or similar. Unfortunately, we do not have a direct feature for this. However, a workaround using conditions may be of help. Check this example: http://www.jotform.com/answers/21374-How-can-I-password-protect-a-form. You can do similar with usernames.

Or, if you're using CMS platforms like Wordpress, you can embed your form using iframe embed code to a password or username protected page.
