    This is a picture of the form.  Country of Citizenship is ALL.  For a moment it does say "United States", then it changes to ALL.

    Hi,

    I was able to see the problem in your Google website when I checked the embedded form.

    But using the form's URL https://www.jotform.com/42254222014944 did not present a similar issue

    So, the problem must be caused by script conflict on the embedded form in the website.

    To fix this, please re-embed the form in your Google website using its secure URL(htps) instead.

    Because Google always requires secure content, it will cause problem if you embed the form using its regular URL (http)

    Thanks for the response.  I did switch to SSL as recommended, however the URL I used in the Google Sites wizard still is http and not https, here is a screen shot of the instructions on Jotform for embedding in Google Sites:

    After I made this change I still have this (minor annoying) error:

    Hi,

    Thanks for updating us.

    To ensure that forms won't conflict anything on your Google site, re-embed your form using iframe embed code.

    Here's how:

    1. Go to your Google Site.

    2. Switch to Edit mode.

    3. Click "Insert Menu" and select "More gadgets" at the bottom.

    4. Select "Embed Gadget".

    5. Paste your form's iFrame embed code. Adjust gadget width and height according to your preference, then press OK.

    This should fixed it.
