Why are the last two pages of my form getting cut off?

I have a form that creates a PDF document.  On the form, there is an icon that says "Click here to Submit" (something like that).  For the last ten or so submissions, the content for the form AFTER the "Click to Submit" button is not appearing on the form, and I don't know why. 

I inspected both of the forms you got in your account and I do not find the icon nor the page breakers that indicate me there are more than one page.
