How can I fill out a form and send the results to an individual person?

    I am an assistant principal in a building. Is there a way to create an observation form, and then quickly send the "results" of the observation to the individual teacher so that they can have the feedback? I only want his or her observation info to go to them in order to protect each individual's confidentiality.

    Hi,

    Yes, you can setup a form autoresponder to be sent upon submission.  You would create an email field in your form to fill in the email for the teacher you would like the feedback sent to and use this as the recipient email in the autoresponder.  Here is our guide on how to set these up:

    This response email will only be sent to the address you enter in your form.

    The forms notification email would still be sent to you for every submission.
