Hi,
We have been responding to your forum posts, but it seems you do not receive our replies through emails.
Anyway, I've fixed your issue with the form email. Your email address was listed into our bounce list due to the following reason:
Result:
domy@sbcglobal.net is IN the bounce list
Reason(s) : 5.3.0 - Other mail system problem 554-'Message not allowed - [PH01] Email not accepted for policy reasons. Please visit http://postmaster.yahoo.com/errors/postmaster-27.html [120]'
I have also fixed your form notification settings. Your sender details are not well configured. You have to select a sender name and email.

Lastly, please use the following tools:
if you're email is in our bounce list, check and remove: https://www.jotform.com/help/262-How-to-remove-your-email-address-from-bounce-list
check your email logs for status and details: https://www.jotform.com/help/293-How-to-view-all-your-form-Email-History
