We've been using the attached form for a few weeks without issue. Today, we started getting issues processing
credit card information.

Your form shows a payment integration with Authorize.net . The integration seems to be working fine as I successfully tested the API Login and Transaction Key .
The CC information should be processed at the Payment processor without you making anything.
