    My form always shows a blank page.

    If I "view form" or "create PDF" - It's blank. I tried different browsers. (Chrome, Firefox and Explorer)

     

    I tried the following in "Setup & Embed"

    - Email

    - Facebook

    - Pop-up 

    - Frame

     

    It always appears a blank page.

     

    Please help!

    Hello Jenny,

    I am able to replicate the issue you are having. It seems your form with it's direct URL also does not display any form fields.

    Upon taking a closer look at your form, I found that you have added several conditions and they have several hide action items. Please check the screenshot below:

    One of your condition seems to be hiding all the form fields from your form. That is the reason why your form appears to be blank. I disabled your conditions and the form fields appears just fine. Please take a look at the following cloned form:  http://form.jotformpro.com/form/42313013745949? 

    I would suggest you to take advantage of our "Revision History" feature to check what changes you have made to your from and the revert back to a working version of your form.
