I am a network engineer at a local ISP in the greater Philadelphia area. A phishing attempt went out to our customers today using one of your forms to collect username and password information from our customers. Do you have a method to report abuse? I understand that you think abuse links are "ugly and unprofessional", but you should at least have information in your FAQ about non-customers wishing to report abuse. If we don't have a reliable method to get forms like this removed, we be forced to block access to your site from our network. I've included the phishing attempt and URL below for your reference.

First of all, we do apologize for any inconvenience this has caused you. We really appreciate your concern regarding this matter. Rest assured that here in JotForm, phishing and any other illegal activities are taken seriously by our administration. In fact, JotForm has its own anti-phishing system which automatically detects, filters, and blocks phishers round the clock. And the form stated above? It has already been auto suspended by our system together with the account associated with it. This means that all of the submissions made from the said form can no longer be accessed by the culprit. However, I will try to submit a ticket for the suggestion of a "report abuse" link or an article to be added on our FAQ/Blog regarding phishing sites and any other related subjects. Hope this helps.

Thank you for reporting that phishing form. I have now suspended the account responsible for that form. You can send your form disable requests to email@example.com, but I would advise you to post them here instead for a quicker response.
