I added a survey and it was working fine until yesterday but now I cannot locate it.

The survey name is IDOE SCN New Pilot Project Opportunity for Procurement of Unprocessed Fruits and Vegetables. The link http://form.jotform.us/form/42303912249147. 

We have opened this survey to our audience and they cannot use it.

Upon checking I found that the form #42303912249147 is already deleted. It is still there in your "Trash" folder. I have now restored this back and the form is working fine now.
