How did my survey delete on its own? Does it need to have an end date?

The survey name is IDOE SCN New Pilot Project Opportunity for Procurement of Unprocessed Fruits and Vegetables. The link http://form.jotform.us/form/42303912249147. 

We have opened this survey to our audience and they cannot use it.

I've just submitted the form and it went through normally displaying Thank you message. As I checked your form, there are no restrictions set on your form. 

Could you provide with more information such as any error message displayed if any and if the issue still persists?
