Becuase You Put A Link In It Leading To An Unsucure URL.

I have tried to access your form using Google Chrome but was not able to replicate the error you reported.
However, your image is not secure at all. This may have caused your browser to detect that certain resources are not secure. Your image's full URL is http://i669.photobucket.com/albums/vv58/lesphoto/facebook-icon.png . Please try to remove the image or upload it through our image uploader (save the image in your desktop then upload it) and see how it goes this time.
Thank you for using JotForm!
