We are having errors with the Box.com Integration on our Payroll Setup Forms. I have
removed and reconnected the integration multiple times but the forms still are not being
pulled into Box.

I could not reproduced the same issue when I made a test of Box.com integration using a cloned of your form.
Please refer to the series of screenshots below when I did the test.
I have setup "Company Name" field as the Submission Folder in the Box.com app.
I made a test submission using my test form. This is the result in my Box.com account.
Perhaps you can try also clearing your browser's cache first to make sure no other lingering cache in the browser is interfering with the scripts that were running on the browser.
Close the browser and then open, then login back to JotForm and retry the integration process again.
