how to insert image to email?

You can insert it on your form notification.

Here's how:

1. Click Setup & Embed then Email Alerts

2. Open your Notification

3. Click the image icon on the top formatting toolbar to insert an image. (**Note that you can only insert image URL. Uploading image is not possible).
