how to insert image

I am not sure if I have understood your question correctly.

Do you want to add an image in your form? You can do that by adding an "Image" field in your form from the "Form Tools" tab. Once the you add the image field, it will open a wizard window and you need to follow the steps. Please check the screenshot below:
