    Hi,

I would advise to re-embed your form again, please use the same iFrame method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Make sure to still use the secure URL: http://www.jotform.com/help/63-How-can-I-receive-SSL-Submissions.

Also, I just want to alert you with regards to the way how you ask for the payment. IT seems like you're capturing credit card details without using our payment integrations. Please note that such form is against our Terms of Use: http://www.jotform.com/terms/. We advise you to use our Payment Integration instead. Since you want to capture payment at a later date, you can use our integration such as Paypal. Paypal allows you to capture payment at a later date, Stripe too or most of our integrations can do this. We please you to change your form within 48 hours to avoid account termination.

    Thank you!
