Salesforce: Separate input boxes for Name

Also, I created separate fields for First Name and last name on my form but the integration with salesforce wizard does not show those two fields. It only shows full name. How do i fix?

Hi,

The integration is designed and works best using the Full Name field under the Quick Tools section. I would suggest using that instead.
