Hi, My user name is PrestigeDocumentServices. What's showing in the emails I send in the "from" area is "prestigedocuments. How can I get this fixed? I want it to show "PrestigeDocumentServices". See the first two emails in this screen shot.

It seems like you're sending or publishing the form via email. It appears that the "From" name is based on the records on your Gmail Contact List. I don't think your form settings has to do with this.
If someone submitted a data to your form, the notification should come as:
New submission: DOMInfo
