I went ahead and cleared your form cache. Would you mind trying it again this time? But first, I would recommend clearing your browser's cache before re-integrating your form to Google Spreadsheet.
So far on my test, I am unable to reproduce the problem even on a clone version of your form.

I'm sorry for the trouble caused to you.
I also cloned your form and I am unable to replicate the issue you are having. I was able to integrate the cloned form with google spreadsheet without any issue.
Like my colleague suggested, did you clear your browser cache before re-integrating with google spreadsheet?
I am not sure what is causing this issue. I am reporting this issue to our back end team. We will get back to you as soon as we have any update from them.

Still no result? Did you try using another browser? Also, I would suggest integrating the form on a non-private or non-incognito browser window.

Still none. Yes, I used other browser, even other computers. I tried it in three different computers, different internet connections and different OS flatform. I still encounter the same error. I tried to integrate it to my personal account in gmail but same error.

The issue seems to be only specific to your account. I have already created a bug report and sent it to our back end team. Upon checking the bug report, I found that it is already assigned to one of our developer. Our back end team is already looking in to yourissue. We will get back to you as soon as we have any update from them.
