    Is there already made forms so we can just customize it i.e. dental forms? Also how do I integrate the forms on my websit?

    Hi ladan,

    First off, I'd like to thank you for taking interest in our service.

    While we don't have a dental form sample, there are quite a few samples we have from which you can build your own form.

    Click New Form > Form Samples, choose a sample form then click Continue.

    You may then add fields to your form. Please click here for more information about form fields and how to use them in a form.

    By the way, I noticed that you do not have an account yet. Kindly register a free account so that you won't lose access to a form you create using our Form Builder and to enjoy a higher submissions limit (100 per month).

    Integrating (or embedding) a form to your website depends on which platform your website is built upon. Most of the time though, using the basic embed methods work perfectly fine.

    Go to Setup & Embed tab then click Embed Form.

    There you will find a plethora of embedding options. Click the link below for more information about the embedding options.
