I got the following message today, the reply email ID was email@example.com. Shall I trust this?

It's probably because of this form: http://www.jotform.me/form/21093400751443. The form is created to with username and password. Our phishing filter is designed to automatically capture this kind of terms in a form and auto-suspend an account. The suspension is actually lifted, but we advise removing the fields to your form to avoid another suspension. It's just, we do not allow it.
