We have a jotform created at a site that we need to clone, but the URL is not recognized.

We have a Jotform at http://www.chambersfamilyfoundation.com/home/grant_application that we need to make adjustments to.  The original account that had access to this form is assigned to a former employee and their personal email address, so password resets have been unsuccessful.  Now, we're attempting to recreate the form on another account, but when I paste the URL of the form location on our website, it says the code is not recognized.  The URL is http://www.chambersfamilyfoundation.com/home/grant_application
