On my JOT Form it says "undefined"

Hi,

At the top of my form for "Questionnaire V2" it shows some text that says "undefined". I can't figure out where this is coming from and need it removed. Any help would be great.

We had a bug in our code a bit ago that has since been fixed.  If you are still seeing the "undefined" message in your form, please clear your forms cache to load the newest code:
