Guest username created Form not showing up on list of forms

I created this form, but it doesn't show up on my list of forms.

Hi,

The mentioned form (and another 1) was created under a Guest account. It seems you have created the form while not logged in to your account/username PPSSpecserv  

I was able to get information that the form was also yours. So, I have now moved both forms

You may need to re-login back to your current account to see the newly added forms.
