Automatically send follow up messages after X days

    Hello. I am trying to create a survey that is a follow up to seek outcome data from treatment. The goal is to have people fill out the form 3 months after they finish treatment. I would like them to get an email when their form is due with a unique ID number already filled in, and then get a reminder email if they haven't actually filled it in.

    I assume I can do this using two different forms, but I'm not sure how to make that happen. Is there a way to make something similar to this?

    I do not think that we can do this with available form tools, since we do not provide the scheduling options. You will probably need to use some custom programming solution or some third party service to send the follow up messages. 

    In theory, you can subscribe the form users to your mailing list, and then send them the follow up emails using the Email Newsletter service. For example, there is an Autoresponder option available in MailChimp, and we have the form MailChimp Integration available.
