Suspended Account: Form not found

I got this error:

I tried to disable and re-enable the form and it didn't work.

Your form has been disabled because it was detected by our phishing tool to be a phishing form. Your form collects information like facebook login etc. which violate our terms of use. 

If creating a phishing form is not your intention then please delete the form and refrain from creating login, signup and forms that collects sensitive info like credit cards. Please review our terms of use.

My form don't collect password/login!

and I have published I need it the same form to be fixed please

all new forms I create is disabled by default also!!!

We are sorry for the inconveniences caused, Our phishing protection system is not very friendly to the Arabic language. Now your account has been reactivated, and existing forms have been whitelisted.
