Upon checking, it seems that you have added the same field "Nama Ship To - ID" twice on your condition which causes the confusion in the system:
Please remove one of the field and the condition should work as expected.

I see, it worked now!
However, I need to add one more field to my conditions here, which is Kabupaten / Kota:
But when I add the field into my existing condition, it says that the term is too large.
So I added a new condition for Kabupaten / Kota field.
But the conditions not working as it should be.
The first condition is not filled properly yet the submit button is already showing.
So I guess the conditions logic cannot be separated, but when I combined them, it is too large.
Can you come up with a solution?

Hello spcregionii,
Thanks for patiently waiting. The Submit is showing because one of the conditions is true associated with the Submit button(for the Kabupaten / Kota field). You'll need to add it to the main condition and tie it with other if statements. Let me help you sort this out with a workaround. It's really easy. Let's do it:
1. Add a Short text field (Kabupaten for conditional Logic) in your form and place all the the required values (that you were trying to set up in the condition) separated by a comma as its default value. Please make sure you add values accurately as in the main Kabupaten field.
2. Then in your condition, set up If statement for Kabupaten / Kota like you see in below:
3. Tie all the conditions with All and hit Save.

However, I need my field as an Autocomplete widget, so the users could select the items from the list.
Is it possible? Thanks!

Thanks for getting back to us. Yes! The autocomplete would still work. The recommendation provided by my colleague would only consolidate each value into a single field. Unfortunately, I wasn't able to test the condition provided by my colleague due to missing information that should be provided on your spreadsheet to form widget.
I would suggest testing this form first and let us know if there's a specific issue that would appear on the new condition.
