My forms are disabled, why do I keep getting suspended?

My forms keep disabled randomly.

The form that keeps setting off our phishing detector is this one:

Anytime a form collects Facebook information along with personal data, our phishing detector gets a bit overzealous.  This combined with your account being fairly new is causing our system to continually suspend your account.  The Instagram intake form you have asking for Instagram username is also setting off our detector.

Removing these two forms or removing the Facebook and Instagram portion of these forms should clear up the issue with our system.
