How do I remove a survey at the end of a form?

In the engineering request form, I used the purchasing request form as a template and it copied the survey at the end and I don't know how to delete this from the end. Please advise.

I took a look at your form but I don't see a survey at the end of your form.

I additionally see this is your purchasing request http://www.jotformpro.com/form/40306400442943 but honestly a bit confused.

What part are you referring to as your "survey" that was copied over?

Can you please elaborate on this more or possibly provide us a screenshot of what you mean? 

Is it this which you are talking about perhaps?
