Please note that collecting of ID numbers through our form is considered as sensitive information as per our Terms and Conditions. 

You would have to do the following to avoid possible suspensions in the future.

1. Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information

These widgets will apply for ID number collection or for ID upload collection. This will make you the sole responsible for collection and handling.

I can see that you already have the Signature widget added however you still need to add the "Terms and conditions" element to your form:

We have unsuspended your account, please make sure to add the Terms and conditions widget to avoid any future suspension on the account.

Once your account has been unsuspended, please try uploading the PDF again and let us know if the issue persist.
