If I understand your question correctly, you want to display your form in click of a button or link button. Is that correct?
There are multiple ways to achieve your requirement:
1. You can add a link button in your web page and then add the hyperlink to open your form in a popup window. Alternatively, you can simply add a hyper link of your form in web page. Please check the screenshot below on how to get the popup embed code and direct URL:
