Show text area field based on yes/no question

I was thinking of asking, "Do you need to add a new store", if the answer is yes, then a text field/fields would appear where they can enter the data.  I would guess they could do the same for remove or update.  And, what if there were several that needed changing?  If they say no, then nothing happens and they continue to submit button.

You can use our conditional logic for that purpose. First, you need to add the question (maybe radio button with yes/no answer) and the answer (textarea) fields.
