    Hi,

The form was created under a guest account, meaning, you were not logged in to your registered account when you made this form. Unregistered guest accounts are limited to 20 submissions in a month, that is why it got disabled unexpectedly.

Import this form to your account so that it will enjoy your account's premium limits, that is, 1000 submissions per month. Please click the link below to learn how to create a clone of the form in your account:

    http://www.jotform.com/help/42-How-to-clone-an-existing-form-from-a-URL

Note that when a form is cloned, a copy is created. This new copy has a different URL from the original one. You would have to use the new form URL or update your embed codes so that new submissions will be stored on your registered account.

As a precautionary measure, always make sure that you are logged into your account before creating any forms. A good practice is clicking the My Forms link so you will see all your forms if you are logged in or no forms at all if you are not.

Kindly inform us if you have further questions regarding this matter. Thank you for using JotForm!

By the way, I temporarily reactivated the guest account so you can clone the form. Please do so before it automatically deactivates again. The reactivation is good for 24 hrs.

Glad to hear it all went well. Please take note of my first response to this thread, especially this part:

Note that when a form is cloned, a copy is created. This new copy has a different URL from the original one. You would have to use the new form URL or update your embed codes so that new submissions will be stored on your registered account.

This is essential so that new submissions will be saved where they are ought to be.

I see, please change the URL to http://www.jotform.com/form/12505019087 and use this URL for cloning. I have already done this for you, just look for the form name "Clone of Grace Worship Info 1" in your My Forms List.

Please let us know if you have any other questions or concerns. Thank you and enjoy using JotForm!

    Hi jgvfbc,

Only the form will be cloned. Submissions and file uploads will not be included when the form is cloned.
