Excel headers with multiple values below?

    Hi!

I need to customize my forms output. As it is now, each field creates a new header in the Excel output. I would like it to give me one header, with all entries matching that field name below it.

For example, if I have a form that requires names, phone numbers, and email addresses of multiple people, I would like one heading that says "Name" and all the names that were submitted (within the same form instance) below it, then another column with the heading "Phone" with all those entries below, etc. Not from multiple forms, but all from the same form.

So the form looks like this:

    Name: [text field]

    Phone: [text field]

    Email: [text field]

    -----------------------

    Name: [text field]

    Phone: [text field]

    Email: [text field]

    -----------------------

    Name: [text field]

    Phone: [text field]

    Email: [text field]

    -----------------------

And the output in Excel looks like this:

    Name              Phone               Email

    Joe Smith        555-555-1212    joe@joe.com

    Suzie Jones     222-222-1234    suzie@gmail.com

    Etc.

Hopefully that makes sense. It's kind of hard to explain in text.

Any help you can provide would be much appreciated!

    Thanks!

    John

    Hi John,

I think any of these widgets fits your requirements. What makes it more advantage is that the fields are dynamic, users can add as many entries and they want by clicking adding more fields button.

    Thanks for your response!

I tried both widgets, and neither works to do what I need. When exported to CSV or XLS, all the information for each entry is put into one cell.

No widget I've tried so far ever produces more than one row on the output. It doesn't help us if all the data is on a single row.

Any other ideas? I appreciate you looking at it!

    Hi John,

Sorry, I think I have misunderstood your question on the first place. I thought you were referring to a table format within your form not on Excel.

Unfortunately, none of the table reports can be formatted according to what you desire even when you use Matrix table. I'm afraid there's no solution for this unless you manually customize the result using your Excel program.

You can also use this workaround on how to move multiple cell values into different columns. http://www.jotform.com/answers/409905

Thanks and my apologies for any inconveniences caused.
