Hello gabriellegoodman,
I did check your form's auto-responder email and found that it does not have correct field names.
Please be noted that if you edit your email template, it becomes dirty. After that any changes you make in your form, it will not reflect in your email template and needs to be updated manually.
To solve this problem, I would suggest you to please delete your existing email alerts and add a new one. When you add a new email alert, it will always display all the form fields with correct field names. The following guides should help you:
Add a new email alert: http://www.jotform.com/help/25-Setting-Up-Email-Notifications
Add new auto-responder: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder
Delete email alert: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder
Hope this helps.

Thank you!
