I cloned your form and made a test and it works fine, please see the image below
The issue could be due to an internet connection from your end. For an additional measure, please try clearing the form cache that may sometimes create issues. Please refer to our Help Guide to get more details on How to Clear Your Form Cache?

Yes. Did the cache already plus history plus cookies. Weird coz it I'm using the same computer testing all these diff forms, so INTERNET SPEED is not a factor. The only difference with this form when we tested it was it was a NEW FORM. So probably the footprint isn't that marked yet so the graphic files are not getting priority in the internet highway.
Anyway, this didn't used to happen in the other forms I've created in the past.
Btw, this was a cloned form. Thanks anyway.

Thanks for waiting, we appreciate it. It appears that our form builder used a broken link when embedding the image in the email notification. I changed the image's source URL in the email's source code and it fixed the issue.
Let me show you how to fix it:
- Use this image source code- https://www.jotform.com/uploads/myhillals/form_files/unf%20ws%20SCHOLARS.62c6f67e364f40.55023140.jpg
- Go to the Autoresponder email settings and click on the source code
- Scroll down a bit where the image src is placed and paste the correct code I shared above, click okay and don't forget to save the autoresponder email setting.
Once done, test it and the logo should not display a broken image.
Give it a try and let us know how it goes.
