I manually checked your account and form and found out that it was auto suspended by the phishing filter.
But the form is not a phishing form. So, I have now corrected this and reinstated your account and form.
You my now continue using JotForm services.

i tried to make other one i used a template from the site i tried to preview it before make any changes it being normal but when i make some changes the problem appeared again !! thanks for reply
