After having issues submitting the original form (see below, "Why is my form getting stuck after hitting the "Submit" button?"), I created an exact clone of the form. I was able to submit it with no problems and received a confirmation email. What do I need to change in the original form to make it go through?

I was able to see this issue on your form http://www.myjotform.com/form/40471746260553
The submit button was stuck to "Please wait.." after I test submitted the form.
It seems the problem was being caused by the NoScript widget on the form.
Please try removing the widget on the form
and then, update also the Custom Thank You message by saving it again.
Please try the suggested process. Inform us if issue remains even after doing this already.
