Form stopped sending notification per conditions

    The form was sending to the email addresses as conditioned and I don't know why it stopped. I have redone the conditions, just in case, and it still does not send. I can see all the forms are being held up within JotForms.

    Hi,

    I checked your notifications and it seems you have two conditionally sent emails but only one notification set up.

    You would need a separate email notification for each conditionally sent email.  Currently the only email address setup is "CAGSStudentServices@ccu.edu" which does not match up with any of your conditions.  You cannot send a single notification to a multiple email address using multiple actions. In other words, you have to create each notification for each of the email address you're sending the form submissions. To clarify,

    Example:

    (Condition1) Send Notification 1  to  email1@domain.com

    (Condition2) Send Notification 2 to email2@domain.com
