Error when submitting the form

    Could anyone please help me solve these problems?

    1) Error submitting the form when I try to test it.

    2) The text of my "terms and conditions" checkbox is not appearing in the form.

    I made test submissions using Chrome, Firefox and IExplorer and was successfully redirected to the Thank you page in all the tests:

    Can you please provide us more information on the error you are experiencing (the submit button freezes, not being redirected to the thank you page, ...)

    I moved the second question to a new thread so our support team can address it better.
