Unable to receive auto-responder email

I keep trying to send a response email with a photo in it but cannot get the autoresponse to send out the email. Any suggestions??

I have tried to test and receive it when i enter one of my emails.  I receive notification that the form was filled out but i never receive the email with the photo in it.

any suggestions?

Hi,

I was able to receive your autoresponder after submitting to your form. Here's a screenshot:

Please try to check your spam folder. Since you are using a custom address, it's possible that the email have filtered as spam. 

Also, I would advise to format your autoresponder using the existing editor instead of pasting content from other source. I noticed your source codes are using MS word codes.
