    Hi David,
I have a form that allows for three separate image uploads. When I receive a submission, I only see the third image in the email I receive notifying me of a new submission and in the Submissions area pointed out by your response.
How do I see multiple images that have been submitted using my form?

    Hello Layne,

Are you having issue with your form "Change Request"?

I did check this form (only form in your account) and it seems you only have one file upload question. Please check the screenshot below:

Please be noted that if you are missing few fields in notification email alert that means the email alert template does not have those fields. This happens when you edit your email alert which makes it dirty. After that, any changes you make in your form, the changes will not reflect in your email template and needs to be updated manually.

To solve this problem, the easiest way is to delete your existing email alert and add a new one. A new email alert will always display all the form fields. The following guide should help you:

    Delete email alert:  https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder

    Add email alert:  https://www.jotform.com/help/25-Setting-Up-Email-Notifications

    Add auto-responder:  https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

    Hope this helps.

Do get back to us if you are having issue with some other form and we will take a look.

    Thank you!
