How do I make a "saved" form point to other forms, but still include original info in the notifier/responder emails?

    First some background to my situation...  We are a School Co-Op working with 10 different school corporations.  We supply physical therapists, occupational therapists, speech therapists, psychs, and behavioral specialists to all the schools.  We have different Jotforms for our people to complete that would then notify other people on the team of a new student, a new evaluation, and other things like that.  Some Jotforms are "2 parters".  They complete one initially and submit it so that we enroll the student in our program.  Then after gathering some other information, obtaining parental consent, and testing the child... about 10 days later they would complete part 2.  Part 2 gets sent to a different group of people than part 1, but those receiving part 2 will still need all the demographics.  

    I would like to cut down on all the entering of demographics time after time.  I know I can create a demographic screen that includes all pertinent student information that could be saved and then continued later.  But... when continued later, I would like that info to pull up automatically and then to lead to a choice of different forms to continue with... For example, a request for evaluation or a transportation form or a preschool intake form part one and then a week later part 2.  I need those original demographics to be included in the email from any other form that branches off from it.

    Is this possible or is it a pipe dream?  If it is possible, can you guide me through this process?

You are also allowing us to work more efficiently so we can use our time to better serve our students.     1.Make a form where you'll gather all pertinent student information, the form is submitted and an email is sent to student, with an edit link to continue later.

    2.The edit link should contain second form link, so this means you must make a second form which should include:

    -All fields from student information form + evaluation or transportation form
