Thanks for reaching out to Jotform Support.
Upon checking the shared folder is available and the form is on your "smartwitness" folder. I also tried to access the quiz form and it is accessible.
I cleared the form cache to refresh the form. Can you please try accessing the account on a different browser? Please try this and let us know how it goes.
