Conditions to show hide multiple form field not working.

Hi I have been adding condiditons to my form but they are not working for some reason. Could you help me please,

When a customer clicks on a different membership type 3 of the 4 fields below should hide only leaving one and when the membership type field is empty the four fields below should all be hidden.

I did check your form and found that the conditions seems to work as expected. Please check the screenshot below:

Please be noted that there is a better way to organize your condition if you want to hide several form fields based on one condition. You should add all form fields which you want to show in one condition  under a "Form Collapse" field and instead of adding condition to show / hide several field you can just add condition to show / hide "Form Collapse" field itself.
